Upper Eyelid Surgery


The eye area is the place that draws the most attention among the facial features of the person. The eye area, which enables the person to communicate with the outside world, also has a thinner and more delicate structure than the skin in other parts of the face. In this respect, the sensitive structure of the most striking part of the face can be deformed as a result of the effects of many factors and cause aesthetic concerns in the person. In this regard, eyelid aesthetic operations are procedures performed to correct sagging or bagged eyelids and to give the face a younger appearance as a whole.


These procedures, which are called blephoroplasty in the medical language, are performed with different methods for upper eyelid deformations and lower eyelid deformations. In this respect, it will be useful for us to give a brief information about the anatomy and structure of the eyelid, in order to understand the aesthetics of the upper eyelid.


Eyelid Anatomy and Structure


Anatomically, the eyelids are responsible for the secretion of part of the tear film. The eyelids are responsible for the distribution of tears on the conjunctiva and cornea, the drainage of tears into the lacrimal system and the protection of the eyeball, by acting regularly and systematically on average 20-30 times per minute.


The eyelids basically consist of the following structures;


 • Skin and subcutaneous tissues

 • Upper and lower lid retractors

 • Orbicularis Oculi Muscle

 • Orbital septum

 • Conjunctiva


The eyelid skin is the thinnest skin in the human body and together with the orbicularis oculi muscle just below it forms the anterior lamella. The subcutaneous connective tissue in the eyelid does not contain adipose tissue.


What are the symptoms of aging on the Upper Eyelid?


Over time, as a result of various factors, there is tension in the eyelids, and with the weakening of the muscles that support them, excess fat collects on the upper and lower eyelids. Thus, bags are formed on the upper and lower parts of the eyelids. A droopy eyelid usually occurs as a result of excess fat accumulation in the upper eyelid. In this regard, the signs of aging in the upper eyelid can be listed as follows:


 • With the effect of aging; The forehead area goes down, along with the forehead, there is a decrease in the eyebrows over the upper eyelids. As the eyelids are sensitive structures with a certain system, they cannot carry this load and in this direction, the eyelids slide downwards.


 • The effect of gravity; It affects the most sensitive and thin skin in the body. The skin on the eyelids is the most sensitive skin on the face and the thinnest in the body, so it is more vulnerable to wrinkles and abrasions. At the same time, the eyelids, which are attached to the highly mobile muscles around the eyes, show a little more wrinkling and sagging with each eye movement. The eyelids, one of the most important functions of which are these movements, become the areas where sagging and wrinkles can be observed quickly with the continuous effect of gravity.


How is upper eyelid aesthetic surgery decided?


The factors that should be examined and evaluated before the upper eyelid aesthetics can be listed as follows;

  • The amount of oil in the lids

  • Muscle structure in the lids


Before upper eyelid plastic surgery, physical examination of the forehead and eyebrows is performed in line with the patient's complaints. By observing eyelid movements, it is determined whether there is any asymmetry.

In patients with minimally lower eyebrows and lines on the forehead, the upper eyelids are reevaluated by applying some preoperative methods. If a continuation is observed in the case of slack in the eyelids, upper eyelid aesthetics is decided.


How is upper eyelid surgery performed?


 • In upper eyelid aesthetics, local or general anesthesia is applied as a result of the evaluations.


 • In upper eyelid aesthetic applications, an incision is made in appropriate sizes and hidden in the fold line of the eyelid, in line with the evaluations made.


 •  In upper eyelid aesthetics, the amount of muscle and skin determined as a result of the evaluations made from the eyelids is removed. By removing these muscle and skin excesses, sufficient tension in the eyelid can be achieved and the old and tired appearance can be eliminated.


 • Depending on the evaluations made and the procedures applied, the duration of the operation may take an average of 1-3 hours.


 • It is the application that requires a good evaluation of the anatomical, nerve and vascular structure of the eye in upper eyelid aesthetics.
